1. Sign a Standard Release Waiver Form to be kept on file at SRA.
2. I must be at least 18 years of age.
3. I will be responsible for the use of the pass code that has been issued to me and for the actions of others that have access and use of this pass code.
4. I will abide by the Rules of the Rink and be responsible for adherence to the rules by those minors for which I have authority.
5. I will supervise those minors under 18 for which I have authority.
6. I am not required to accept the responsibility of minors who are present and unsupervised at the rink.
7. I have the authority to demand unsupervised minors leave the ice and to report their unmonitored behavior to the office.
8. I will not share the pass code with anyone under the age of 18, who is not on my SRA membership.
9. I understand the pass code is to be used only during posted open skating hours. Use of this pass code at any other time is prohibited and may result in loss of rink privileges.
10. I understand that use of the rink is limited to members. Use by others is prohibited without advance approval from the Ice Committee or guest waiver and fee.
